Designed form won't install for only one user

I have created an appointment form to reserve a conference room. It is working for everyone in the company except for one user. When he opens the form the standard appointment form opens, not my form.

Any one have any ideas on what is going on here. The one person having the trouble is the one that sets the conference room up so he needs to know how the room is to be configured.
