Have you recycled the carriers? If you can connect it to a new cable. If you can on a weekend run a short piece of cable punch it down straight on the port and see if it still happens. This is assuming that it is not a 24x7 operation and the problem is frequent enough to happen within 48 hours or so. Have you tried a differnt power supply DSS everything. Since it a release 6 and this was working my guess is that some piece of hardware is causing this. replacing 1 piece of equipment at a time might help you find it and I do mean everything including line cords and handset cords...... Can you put it on a differnt carrier?
Replace the operator that has a wig with one that doesn't. Or install a anti static mat on her desk. Wigs are famous for creating huge static charges that play #@$%^ with electronic devices.
Replace the operator with great care though, so U don't open yourself up to a huge lawsuit.
