Database Error when forwarding

When attempting to forward all calls I recieve the text "Database Error" on the phone. Forwarding works on all other sets. I have deleted the line, removed it from the unassigned DN's and re-added the line and the problem still existed so I restarted the Telephony Call Dispatcher and reinitialized the database with DBLhelper and the problem still exists...any ideas?
 
You have done everything I would have. How about resetting the phone factory defaults and then adding it back in to ccm?
 
The Database Unreachable message is displayed on the IP phone when the subscriber CallManager has Structured Query Language (SQL) connection or replication problems with the publisher CallManager. This message is triggered by a function or feature request which requires the subscriber to access the SQL database on the publisher. Check for replication errors and basic network connectivity. Refer to Reestablishing a Broken Cisco CallManager Cluster SQL Subscription for more information.

 
Check CSS in call forwarded calls in directory config
for the dn.
 
Reboot both servers. Pub then Sub. See if you still have the error. If so, double check the SQL subscriptions as rroblez said.
