Can you tell me if a CSU (120A)is required when installing a TN464GP DS1?

A csu of some type is required on circuits that go to a network provider. 120 is but one type.

program on ds1 form as integrated if you use a 120a csu.
program on ds1 form as other if you use a csu other than a 120a.

Do you know if a TN4654GP has an imbedded CSU?

120a csu plugs into amphenol connector on the slot behind the tn464 or tn2464 or tn767

only the media gateway tn710 have built-in csu modules.

Can I use a standard straight through Cat5 cable or does it need to me a special cable?

ds1 uses a shielded transmit and a shielded receive pair.
This cable should connect to the csu and go to the demarc which is most likely a smart jack installed by the provider.

This is not cat5 network cabling.
