We have a IP 403 that has started crossing lines.
For example, if I have 2 separate external parties on hold, and then I transfer a external party to someone internal.....both external parties get's bridged when doing the transfer to a internal extension.

This has been happening enough that fd is now noticing this.

What would be causing this...

An old and tired system probably, one that needs retiring :)

We've only had the system for about 6 years...
Prior to that , we had a Partner System that had grey hairs but ran without any hiccups.

I'm inclined to say it's time to upgrade the chassis, but what would be causing this.....just short of old cranky system ;-)

I am gonna guess and say your software is about 2.0 something??? If it isn't on the latest for the 403 which I believe is 3.2.69 try upgrading..I have seen this issue years ago

6 years of running 24 7 is not a bad effort, I would bet you don't have many if any other pieces of IT equipment to match that. But short of the lines causing the issue it's probably just worn out hardware :)

I am with Andy on this one, 5 years is the life expectancy of a system and then it needs to be upgraded via firmware or what most manufacturers want you to do is buy a new system. 80's you could plan on 10-20 years of system life if no lightning made a mess out of your circuit boards but nowadays it is all so fast paced that 5 years from now the features you are using are outdated and you want new ones, or at least that is what all the sales people tell you, you need new features, great ROI bla bla bla
but getting to the 403, 6 years are not bad for it, try upgrading as NYSTECH suggested if you are not running the latest version, also a hard reboot sometimes solves weird stuff like that, meaning unplugging the system completely and wait for at least a minute to plug it back in (the power supply not from the back of the unit I might add) give that a shot

Google it you damn kids

But also a reboot can kill the system when it is running fine for years :)

BAZINGA!

I'm not insane, my mother had me tested!
