creating half page foldout in ID CS2

Hello, I just realised that there is no way of creating different sized pages within a document. Now I need to create a foldout of 100 mm to righthand page of a spread and I´m confused. my books and documentation (or ID help)do not cover this situation. Increasing bleed is one way I suppose, but is there any other ways to go?
Any help is greatly appreciated

You could create a seperate document for the fold out.

Then combine them in the PDF if you have to have it in one document.
The Print shop they will deal with it at their end. Because those sort of things generally get printed seperatly to the main text anyway.

Thanks for the excellent reply. We´ll do that.
If my memory serves me right, Freehand was able to handle different sized pages, but in the printing stage that would propably cause problems of it´s own.
