Creating a Macro or Script on Adobe programs???

Hello,
I'm trying to figure out how i can creat a script or a macro that will enable me to prompt something and then have it show up on a photoshop document.

In other words, when the file is opened, a question box should pop up, asking a series of questions where the user can imput information and it will automatically place the information on the .pdf file.

Im not sure if this can be done with photoshop, but any help is greatly appreciated

Can applescript work on Windows? Or is there a similar program that will work for Windows?
