Create Multiple inputbox

Hi all
I'm new to this forum.
There's a problem I'm having by defining the inputs for a procedure of automatica calculation. I need to let the user define 5 values from a single input box, these is not feasibile in VBScript and I found that can be done by using an HTA application, but there's no sample for my requirements. The inputbox is a vrey simple one: it contains only the name of the variable, a box where to insert the value and a button to assign it. Then I need to take those values from the HTA and use them in the VBScript i'm writing.
Can anyone help me?
