Create a Reservation/Event Reply Form

I am new to programming Outlook forms. I have experience in Access VBA but none for Outlook.

I need to send a form to a group of people getting attendance dates for the event for the participant and if any quests. Also need to get responses to activities available to see if participating. How do I do this? I have read notations from help, etc. that suggest using a previous form and then editing which I attempted and got no where because none of the templates would let me edit them nor did they fit the format I needed.

ANY help would be greatly appreciated.

Do you mean you're trying to send out a meeting request with date and time. You also then want to keep track of who can attend?
