Cooking DS1 Boards

Anyone ever experienced a problem that causes DS1 board failures. Have a switch that is going on its fourth board in 3 weeks. Im thinking CSU at this point because i have no power alarms on the switch. We did change slots. We were thinking it was LD provider but customer as luck would have it changed to new one and same trouble.
 
I would be inclined to lean towards a power problem.
Make sure the system is properly grounded.
 
If you are using a 120A3 style integrated CSU odds are it is the CSU. DO NOT use any more boards until you call the AVAYA hotline. Give them the serial # of the 120A3 CSU. This CSU is about the size of a pack of smokes and resides on the back of the carrier directly attached to the rear of the slot. You need either a 120A3A or 120A4.
 
Any PKT-INT errors associated with this problem?

What type of TN board is it?

You hit it rite on 120a3 serial #'s 01dr09 thru 02dr04
are bad. The 120a3a is the fixed version. Also TSO said they had 8 120a4's yesterday that failed so beware and watch for the QPPCN. Thanks for your help.
 
The QPPC for the 120A3 ICSU's is 1359B

I have also been told that some CSU's that date before or after may also have that problem. The serial # range is not written in stone. But typically they fail within a month of service but not always.
