Are you using a full version copy of Adobe Acrobat or just the Acrobat Reader?
If you are using a full version you should be able to convert the document to *.rtf (I'm away from my actual computer so I don't remember the exact memu/location).
If you are using Acrobat Reader you can change the view to continuos sheet view then "Select All" and then paste into Word. This method works however you will need to clean up the document.
With regards to Word/Bookmarks and keeping them in PDF I think you can only accomplish this with a full version of Adobe Acrobat.

I am using the full version of Adobe. I tried saving the pdf as an rtf, but this doesn't keep my tables, etc. I saved as an rtf, reformatted it in word, and made my changes. Now I want to convert to a pdf. I setup all of my bookmarks in word, but they are not there when I open up the new pdf.
