Conference Callers Dropped

Has anyone seen issues where confernce callers are dropped out of their meeting. We have checked with ATT and they are showing it as a normal hangup. This only happens to some callers. We are on a S880 running R016x.00.1.510.1. We have no other complaints of dropped calls except for conference calls and not everyone gets dropped.

What type of phone? On 9620 the mute and speaker key are close together and I have discovered people will hit the speaker key instead of mute by mistake and hang up.
