Conf. callers can not hear each other

I have a Norstar 4.1 MICS with a PRI. Suddely, when using the Conf. feature the conferenced parties can not hear each other.
It seems that the inside set that initiated conf. can hear each party but the 2 outside parties can not hear each other.
Customer added me into a 3 way using the speaker phone. I could actually barely hear party. I requested customer pu handset and was not able to hear party at all.

I would upgrade your software to the latest. I also would put in a NTAB2666 Norstar Audio Conferencing Unit.
