CM Special App suddenly stopped working - SA8888 (Per Station Music On Hold)

We had SA8888 (Per Station Music On Hold) enabled and working for several months on CM 5.2. It has suddenly stopped behaving. The Special Application is still enabled, and we still see the "Music Source" field on station forms. However, the Tenant's music source has suddenly taken control again, disregarding station specific settings.

We're considering trying a "system reset 4", but I figured I'd solicit advice here first. Any suggestions?
 
Do you see anything interesting in a list trace of the phone call?


Are the affected calls dialing the DID/extension directly, or are they coming from VDNs/transfers/other sources?
