CM 5.2 MBT Utility Server and phone firmware update

I am trying to use the Utilities server to perform firmware updates on IP Phones. I can upload and activate but they do not show up in the directory until after I try to install on a phone. All updates to phones fail. Why? Is there a way to find out why the update fails?
