CCM Fallback issues (SRST)

I am having trouble with a couple of remote sites failing to revert to CM-Fallback mode (SRST).
I have 50 other sites that are fine, but for whatever reason, 2 of them are having issues.

My set-up is a CCM-Pub and CCM-sub release 3.3(3)sra4
I am using a Cisco 2811 at the sites in question with fxo module.

I have added the config, so if anyone can maybe see something I am not seeing. Any help is appreciated greatly.

I did forget to add the problem, Sorry
Problem we are having is when the system reverts to SRST mode, phones fallback fine.
We can receive calls, but when you try to make a call we are getting re-order tone immediately.
It won't let you pulse any digits for outbound.

Version of CCM is 3.3(3)sra4
Router is a Cisco 2811 IOS ver. ("flash:c2600-ipvoice-mz.123-11.T.bin")

Looks like you use 9 to get a dial tone.. so what about adding the "access-code fxo 9" to the call-manager-fallback command.

That should give you a dial tone at least.
