Catalog Failed

I am running Backup Exec 8.6 installed on NT4 and catalog will not work with the tape drive(Exabyte). I installed Backup Exec on another machine and did a catalog using hard disk instead and it work fine. I have done all the updates and still not luck. Anyone been through this mess before?

Additional error message:

Storage device "Exabyte 1" reported an error on a request to seek to a logical media address.

The request could not be performed because of an I/O error.
