Cannot transfer outside line to internal extension

This is a secondary Secretary phone. When a call comes in on an outside line, if you attempt to transfer the call, you get a rapid busy signal. But calls that originate from the internal lines can be placed on transfer with no problems. I am not a PBX tech by any means, but have been tasked with trying to find a solution to this problem. If you need any information on the pbx system or any details to help me figure this out, just ask. I might not know what you are asking for, but if you tell me where to look, I can find it. Have three volumes of the neax system manuals sitting here and can't make heads or tails out of this.

i have a customer who will receive a call, when she transfers the call to the requested ext ( that ext is forwarded to there home #) The receptionist can not release the call from here console phone?
