Cannot answer ringing phone

I recently installed a Norstar CICS system.
The customer complaints that 3 to 5 times a week, extension 221 (M7310) rings like an incoming long distance call. The call does not ring at any of the other ringing sets, and cannot be picked up from any of the sets, including extension 221.

What display is in the window when this happens? Does this CICS have ISDN on it, or just normal loop start lines? What error display do you get when you try to pick the call up?
Sounds like something is falling to "prime", we just need to look at the clues to determine what it is.
