Calls not ringing at rendom

IPO500 R9.1.5 PRI. Have a hunt group of 14 sets (1416). At random calls will not ring to some extensions. Re-groomed whole configuration, re-formatted SD - nothing helps. While watching trunk activity I can see that certain extensions are not being alerted at times. I am about to get fired from this job. Please help!!!!!!!!!!!!!!!!!!!

by the way your monitor trace is useless you need to start it at defaults and then place a call. Then post it with the callerID info of the person placing the call that failed so that we can traces it.
