calls not hitting pbx

I am working on an option 61 and i keep getting messages that my call cannot be completed when line 1 is busy. i just want to be sure i am not missing anything, i have 4 lines all marped on 1 phone and fdn and hunt both set to go to vm and lhk set to 3 and am running out of ideas. please see below and let me know if i am missing anything. any help is appreciated.

check with the provider.. the provider sees that call and does not allow the second call.. that would take a second line from the provider.. the recording is from the provider, they don't have forwarding on there service

great, then everything on my phone looks good. i just wanted to be sure before i went to calling them.

DN's are not lines, if you have 4 lines are they set for AUTO TERM on the RECPT phone EXT???
