Calls not disconnecting

Has anyone experienced the following, call goes out a local co trunk, then the call appears to tie up another member in the same trunk, when the caller hangs up the PBX never callapses the call. see examples below. Any Suggestions?

I would check to see if your wires are mixed up. You might have the "tip" from one line and the "ring" from the other.
 
We'll look into the wiring. How does this explain calls made at time we are closed. After midnight? 2am, 10 or 11pm.We are a Financial institution with very strict security measures. We know there is no on in the building making calls.
 
The calls are all generated from 3083, I would check what that number is and start from there.
 
3083 is one of our DID numbers assigned to a phone in the lunch room.
 
at 2 or 3 in the morning? and closed? We're a Financial Institution w/pretty strict security...anyway I changed the ars table to deny that specific number..we'll have to keep an eye on it. Thanks for the input.
