Callpilot mini not responding

I have a callpilot mini installed on an opt 11c rls 25.40B. It has stopped functioning, after it is rebooted all the LEDS keeps blinking in sequence from 1 to 4. I removed the PCMCIA software to my laptop and i am able to see all the software files. Does anyone know what i can do to determine the source of the problem?

Is there a tool i can use to reformat the flash card an reinstall the software?

Did you pull the card from the PBX and re-seat it?

Check the pwr supply. The power adapters were known to die on callpilot 150s. The LEDs would appear to be still on but system wouldn't come up. May be u have a similar problem.

I have tried reseating the card and changed the power supply, but this did not help.

could then be an internal h/w failure which means replacement.
