Callers cannot hold or transfer intermittently

We just installed a 12 controller cluster consisting of MXEIIIs. There are four admin groups with three controllers each and 6 users from two controllers in the same admin group have reported that often times they are unable to place a call on hold or transfer. Nothing happens when they try. Most are trunk calls over the PRI but a couple have been internal. Any thoughts? Thanks in advance!!!!
 
Also, it is happening to a mix of superset 4025 digital and 5330e IP phones.
 
I will bet big dollars that they are trying to put a console on hold or transfer a console.

This is not possible by design.

Thanks for the reply!!! There is a dispatch center that has roughly 6 IP consoles deployed, but they are saying their communication is directly with patients, which tells me they are speaking direct with outside callers. Its possible they came in through the console station and then transferred in. Would the console still come into play in this scenario? The internal hold failure problem very well may have been. I will dig deeper on that.
 
The system will not allow hold or transfer if it has run out of DTMF receivers.

If this was your issue, then you would have other complaints.

Still betting on the console regardless of what they say.
