Call one ext # and get a diff extension?

I'm new to IP Office and I have a odd occurrence. V9.1 Office. Every so often someone will dial a extension and get a completely extension. Doesn't happen all the time but I believe it is happening daily. x29206 called x29214 and got x26212. Records are attached. X29214 isn't fwded and is set to go to vmail if it isn't answered. Ive called x29214 and it goes to her phone and then ends in Vmail. Hoping the records tell something,

Run monitor with the call and targeting filters on. This will tell you the digits dialled, where the call is targeting and why.

It's probably internal twinning. This is found under the mobility tab for the user 29214.
Don't forget that twinning can be applied with a time profile. That might be why it's happening at certain times of the day.
