Call log shows almost all numbers "Unavailable"

Here's an odd problem I have not ran into before:

Customer is running on an IPO 406 with 4.2.14 on it. I just received a call in which a guy is saying that almost all of the numbers in his call log are showing up as "Unavailable." This happens with both internal and external calls.

Anyone have any clue as to what might cause this?

call log in the phone?
which phone? (firmware release on it)
what type of lines?

Yeah, the call log on the phone.

It's a 4625 running 8.016 with a PRI.
 
Sorry never had that happening but we do very few full IP installs and I have not even a 4625 here to test for it.
Does the caller ID come up in the screen when the call comes in?

Seen this on 5621 phones, however, the set up was slightly different (and Avaya acknowledges a known problem):

Using Line Appearances (analog trunks)
Call Log at the phone the call was answered at is fine
Call Log at all phones the call was not answerd at shows unavailable

My suggestion for you is to open SysMon and watch to see if caller ID info is making it that far. You will then be able to determine a handset problem or a provider problem.
