"Date-Picker.js"
Now, on your forms use this code
Use any image you find suitable for a calendar that you can place next to the field where the data selected will be entered. and attach this piece of code to it, replacing the name of your field.
'-------------------- And lastly put this code within the head of your document
<script language="JavaScript" src="date-picker.js"></script>
There you go.
