Having to install 3 BM32s over a 1616 phone, are them all powered directly from the IP Phone (through POE) or do they run only if locally powered with the external power supply ?

The KB states: “Please note that an external power supply is needed when connecting more than one BM32 to a 1616 telephone”, it means that a POE switch has not to be used at all ?

POE can be done if you do not use a lot of buttons.
It can be too much and thus why you need an external power supply.

BAZINGA!

I'm not insane, my mother had me tested!

It's always one or the other, PoE or injector, but not both.
Peter's right. An 1151D1 injector might suit here. You supply the power cable, it does not come with the kit.

Kids, Kids, It's Avaya. You're both right.

POE will power no more than 1 expansion module

If you need any more disable POE and use the local power supply, as holdmusic34 said don't mix the two
