Best way to remove Caller App?

We have some corruption on a subscriber and would like to remove the caller app and mailbox in order to start fresh. For some reason, an incorrectly typed extension on one of the nodes was 'memorized' by the app and won't let it go, even after we had removed any reference to it.

What is the cleanest way to remove the Caller Application?
 
did you remove the association before you tried to un-deploy it ?

Ken, I just stumbled on to that element, and the removal was a success! Thanks for your response, and for the value you bring to this forum!
 
So after you remove the association and undeploy the caller app, how do you delete the caller app from the program?
 
Go to Deployed Applications\SERVERNAMEvmdom. Highlight Name in right window, right click and select Delete. Then Undeploy caller application from the domain. Follow the Wizard.

Step 2 - Delete the Caller application file from the folder in C:\Program Files\Avaya Modular Messaging\Caller Apps.

Step 3 – Delete subscriber from MSS. (no screen shown).
