The system is patched current. There are certain extensions that are not getting message waiting indication. One user had 10 new messages. Has anyone seen this?

Got reports of the same issue last week. Its also patched to SU11. Reboot has cleared for me, well so far so good. I am half expecting it to return though. As cook pointed out, this was supposed to be resolved by a patch that was released a while ago.
Actually is was the last patch the DD mentioned that supposed to have fixed:
Software update BCM450.R600.VOICEMAIL-62
----------------------------------------
1. The Message Waiting Indicator (MWI)does not display intermittently
This improper behavior is caused due to delay in execution of semaphore
requests issued by voicemail to kernel. This update will correct the issue.

Guess it doesn't work as another post or 2 I think stating same.
This issue has happened to the wrong customer. They had a BCM400 that worked fine. Due to the Avaya acquisition we proposed a BCM450 and IP500. They chose the BCM450 and now there is this issue.

I have that same issue with a Bcm450 R5 fully patched. The msg waiting lamp fail every few months and always right after a scheduled backup.
Scheduled backup is programmed "weekly" but Msg waiting notification fails only every 3-4 months.
A restart of the voicemail service fixes it.
