BCM450 release 6 -SAFE OS- upon every system reboot

I have a new BCM450 release 6. I can't get LAN 1 to work. Everytime I make any change in programming requiring a "reboot", the system ends up in SAFE OS. I have to use PuTTY to "Transition in the main OS". I have the license applied and I have the SU from June 2011 installed. I have replaced the Hard Drive with a new one, and the issue is still present. Is this a TRAY issue or something else?

we had the same thing i believe we had to replace the bft
