BCM no free lines state of the line is showing connected

I am pretty sure I know how this is happening and can recreate it but have not figured out how to fix. Taking multiple lines can occur when callers press 0 to go back to the main menu. For some reason it takes an outside line and calls our main number to get the Auto Attendant. When someone calls the office1 BCM and does that then the lines do release after the call but if they call from office2 then it does not always (sometimes does) release those lines.
 
What is office 1 and 2 mean?

The issue is when people press 0 it conferences back into the system?

OK, so we have 2 bcm's connected with voip. Office 1 has pri office 2 has 4 analog lines. When a caller calls office 2 the call is transfered to office 1, if the caller gets VM and presses 0 it goes back to the AA. During this time the call is taking up both an incoming and outgoing line for each time the caller presses 0 and the BCM in office 1 will not release the line and it shows connected for upto 30 hours till we reset or unplug the PRI line in office 2. And thoughts
 
-To get this out of the way do you have MCDN key codes installed for "centralized voice mail"?
-Why or how is the caller transferred to Site A (office 1) when they are trying to call Site B (office 2)?
-What is the attendants DN set to at site A?

Sound like site B is forwarding to Site A and Site A is forwarding the call right back to Site B

You need to spell out the programming setup....completely.
