BCM Monitor missing provisioned lines 1

I have a BCM450
We have an intermittent issue where incoming calls get a message that "all circuits are busy - please try the call again later"
When this happens we can usually only have one call in at a time. Outgoing calls are not impacted at all.
We have 2 PRI's, one for incoming and one for outgoing.
They are set up as Trunk type:pRI, Protocol:NI-2, NSF ext:None, B Channel Selection: Descending Seq, Clock source: Primary external. the outgoing clock source is secondary external
Incoming PRI has lines 009-031 provisioned although all call incoming cals start on 019 in BCM monitor. They then skip acouple of lines then grab 022 and then on down.
When the All circuits busy problem starts you see a call on 019 and then 022 flickers with a call tehn it disappears.
We can correct the problem by disabling then re-enabling the PRI.
Not sure if its part of the problem but the incoming PRI is missing lines 026,027 and 028 even thoughthey show to be provisoned.
We have swapped out this particular PRI card and have al;so swapped the PRI circuits around and always had the problem stay on the incoming side.

the carrier never shows a problem and we have verified all the PRI settings match the carriers.

Do you have the latest patches on it? There initially was this exact problem that was fixed via a patch.
 
The need for latest patches was the problem. I applied Nortel's latest patches and problem vanished.
