I have a BCM 400 3.7 with some 50 IP 2002 sets on PoE but the BCM appears to crash about twice a month for no reason - basically reboots itself. I caught it once and used a digital phone on it to call an IP extension, and the digital phone pulled dialtone and called with the resulting "not in service" for the IP sets. Has anyone seen this before?

Check the revisions on the BFT and the MSC. I have seen the rebooting as an early sign of BFT failure.

Check cooling, as the BCM is a PC. Open and remove all dust.

What version of BCM are you running? I know you say 3.7 but was this an upgraded machine? 3.0 to 3.5 to 3.6-7 may be your troubke as all 3.0 to 3.5 have a service advisory on them for the BFT and MSC boards. Back it up now while you still can and write down the SYS ID as you will need that to regenerate keycodes later. Good luck!

It's running on 3.7 but not an upgrade. We only just got the machine new (if something with NT 4 can be called new). The system is from mid-January 2006. Is there a way to see the upgrade history that could reveal that it was 3.0 and has been upgraded?
