BCM 450 IP sets not transfering calls

Have a strange one here. A BCM 450 with IP trunks (site A) and MICS with Nortel VOIP gateway (site B) and BCM 50 r3 with VOIP trunks (site c). We have the sites configured for direct intercom dialing across the VOIP trunks. IT all works fine except for this little issue. When site B intercoms site A IP sets and the IP set transfers the call it never transfers and the site A IP set can still here the site B caller? Now if site C calls in using the same process the calls transfer fine? Also if site B calls any digital set in site A the calls transfer fine? The system is patched through July releases. Any ideas?
 
Two things spring to mind here, but it's probably wrong!.

1) Remote Access Packages. Have they been checked to allow full interaction in case different routes have been assigned?.

2) How many V.O.I.P. Trunks do you have on your systems?. Are there enough trunks available to allow the call to be transferred back?.

Packages are fine and there are 8 trunks of which i monitored and only 2 were used.
