BCM 400 with 4.0

I have a customer with a BCM 400 with 4.0 software. We went out last week and applied all the latest patches from Nortel. They called us two days later saying they could not receive incoming calls on their PRI. We had them disable and re-enable the T-1 card but no change. After having them reboot the whole system, everything worked fine. They were trying to blame the patches for this and we assured them it was just a fluke. They called again yesterday with the same problem and again kept saying the patches were causing this. When they were down and the customer was looking at BCM monitor, they saw all 23 channels on the PRI greyed out (lines 91-113) When they were making test calls into the system, they would see their call hit line 95 for a brief second, it would flash blue and blink real quick and then they would either get a fast busy or "all circuits are busy". I still want to say it is a fluke with the patches and say maybe a carrier issue, but the calls are showing up on BCM monitor, just not passign through. Has anyone else had issues like this before? Any help would be appreciated.
 
Which SU patch did you install? I had the same problem with SU 18. Problem was fixed with SU 19
 
Was it one of the latest update patches that were applied?

if so, Nortel released a new core patch to overcome this issue, I believe the core patch is 261
