Basic Comment Form needed...

Hey group. I need a basic Flash Comment form.. something with a field so people can enter a comment about the site and hit "send" and it sends me the info. Simple (i hope)
Anyone have something like this laying around the flat?

Can't be sent automatically to you, if you don't use some server side scripting... Is that what you had in mind?
