While an extension is off-hook on a co call, can another extension voice call through the speaker without muting or putting the call on-hold? I have a customer that has an Executone IDS with this capability and I have not been able to find a phone system with that works the same way

No, Off-Hook on Busy, but there is Voice Interrupt on Busy #312. The announcement comes through the handset, add a button to allow talk back.

BTW, Merlin Legend, with MLX sets, offers Off Hook Voice Announce.
