Backups failing B2D used

I am a consultant whose clients backups keep failing every Tuesday. They are using Backup Exec 11d with a High Rely 7 SATA disc for the B2D option. Backups run everyday to each disc. Someone swaps out the disc each day after a backups. I am not a Backup Exec guru by any means, someone else set this up. I think I determined what the problem is, as you can see from the screen shot the Tuesday backup lacks the "FLDR00000x" like all the others have. How do I add that? What is it? Thanks in advance!

I believe that is the folder the backup is supposed to save to. You specify it in the job properties.
