B149 - Making Conference Group Calls

We have a B149 conference phone hooked up to an analog port on an IP Office Essential edition 9.0. We are able to make outbound calls and conference together several outbound calls together (found this thread that helped:

However, we are not able to make pre-programmed conference group calls. On page 11 of the guide below it shows the steps to make a conference call but this is not putting all the calls into a conference.

you changed the flashhook settings on the phone to match your locale? In my locale, it needs to be set to 400mS. You also need to turn call waiting on in the Manager for the user so it can receive a second call if desired.
