Avaya Expansion module does not work DS16B

I have a DS16B module that was working once upon a time. It came back from a site and none of the IP500 chassis that I have in house can see it. I applied power, the light on the front turns solid red then goes off. Never comes back. I have the DTE port connected to my laptop and it says repeatedly:
TFTP Poll Sent
TFTP Poll filename = dsaupnV1.bin

Then after a few minutes it changes to:
TFTP Poll filename = dsaupnV1.bin
TFTP Poll Failed

Anyone have any suggestions? Is this module doa?

Do you have the Manager TFTP server running with the file available? That is what it is looking for.

Yes, I made sure that Manager is open and the file is located in the Manager folder. I sends the TFTP request about 9 times on the 10th time it comes back immediately with a TFTP Poll Failed.

what does the Manager TFTP log say?
have you configured the OpenOffice to look at the correct PC

At no point does the 16B module, which I have a serial cable connected to do anything other than continue with

Well, this sucks looks like I have a 16B module that has issues. I went through procedure step by step, when I got to the step where the IPO is off and I power on the 16B module and press the ESC key till I get the Loader message; it never gives me the Loader message, nor does it show anything loading in the TFTP Monitor screen. Here is what the hyperterm screen shows when I turn the unit on before turning on the IPO.
