Avaya 500AM headset adaptor help?

Hi, I've got a few Avaya 500AM headset adaptors that I would like to use, but I can't find any documentation on them. In other words - how do I hook this thing up, and what can it do?

I've checked everywhere on Avaya's site and elsewhere on the web, but have so far come up with nothing. The closest I've come are the Terminals & Adjuncts reference manuals, but they only mention the 500A - and even then, there are no detailed instructions.

Avaya unhelpfully chose not to label any of the sockets (well, aside from the word "headset" moulded into the front!). The front of the unit has a plug-prong socket, and an RJ10 (handset-type). In the rear is a 6-way RJ11, an 8-way RJ45, and what looks like a "miniature" plug-prong socket.

Any help is greatly appreciated!! Many thanks in advance.
 
Run a D8AC cord (ones I use have blue rj45 connectors) from the back RJ45 to the bottom of the phone. (84xx have it labeled as speaker on the bottom of the phone next to where the line cord connects)

They need external power. We use the power units that go in line from the wall to the phone. Be sure to use a 4 pair cord from the power unit to the phone, power is passed on the brown pair.

Have users that use both the prong and rj11 on the front, most like the prong over the rj11.
 
A couple items to note:

1) The power supply is an 1151A1 Power Unit.
2) You cannot use a regular RJ-45 as the interconnect cable between the phone and the amplifier. It has to be the Blue Tip cord mentioned by GMinTX above. If you need a source for them (they can be very hard to find) I can spin you in the right direction.
3) I believe (and I could be wrong about this) that the 500AM will only work with 84xx series phones.
