avaya 1416 firmware upload error

I have New 1416 phones that have an error message of:
NO APLLICATION FIRMWARE
FWIMAGE FILE INVALID
DOWNLOADING 077KB

I have 3 phones that do this, the others download there firmware OK.
I've rebooted the 500v2 and manager is open and running. Problem persists with these same 3 phones.

plug them directly into the ds port, I have had issue's with 1416's not updating through the customers "new" wiring, but then they updated fine when directly on the DS port.

Also, have had some phones that are bad out of the box and will not update no matter what.

This problem tends to be the combo card, upgrade them from another card if possible
