Attendant console caller ID issue

We installed a new PRI circuit and our attendant console phone is only seeing a city and state on incoming calls. Every other phone is able to see both the caller's name and their telephone number.

Does anyone know what else I can check to make the attendant console see the right caller ID information. Is it something else in public-numbering that we may need to tweak?

We know the carrier is sending the name and number as we have verified that with some debugging tools.
