ASBCE 8.1.3 to 10.1 upgrade fails 1

Hello all - we are attempting to upgrade our Avaya SBCe /Portwell CAF-0251 from 8.1.3.0-31-21052 to 10.1.0.0-32-21432. We have been following the upgrade procedure exactly as it mentions, but everytime the device reboots to finish upgrades, it stops and gives the following error:

We have uninstalled all 8.1.3 hotfixes before attempting the upgrade, as well as ran the pre-check uber utility, which said Pre-Upgrade Verification Test Pass / Upgrade can be proceed further; but yet evertime we attempt we get the same result as above, and upgrade will not proceed. Does anyone have any suggestions? Thanks,

Just an update - We continued to have problems getting the 10.1 upgrade to go, so we ended up performing a fresh install of 10.1.0.0, and then migrated/restored from a backup. This method worked perfectly, and so far so good.
