I would suggest installing an SMTP server to filter port 25 in front of the Exchange server. There are different brands including a Linux/GNU version, but I usually use Mail Marshal on W2k Server as it nicely works with different AV vendors, includes the SQLserver to track and store all e-mail, and is pretty easy to keep updated as the spammers develop new tricks. The firewall routes the mail to this server (doesn't need to be a big powerful box) which scans then passes messages to the Exchange server (which routes outgoing mail to the marshal before sending to the outside world also.) This has been very good for my customers.
