Analog 30 expansion module stuck flashing red

Hi there,

I recently shut down our IP500 using File->Advanced->System Shutdown->Indefinite because I had to move some power cables to make the fire marshal happy. When I powered it back up, one of the four analog voice 30 expansion modules is stuck rapidly flashing red. Everything else came up just fine (SIP trunks, digital phones, other analog expansion modules), but the Status program shows "not present" for the module that's flashing (instead of "Phone 30 V2"), and there's no dial tone for phones connected to this module.

From what I can tell, the rapid flashing red LED indicates it's in a shutdown state. I've tried unplugging and re-plugging the power to that module, rebooting the whole system, shutting down and removing power for the whole system, and I've checked the UTP cable in the back as well. I didn't make any changes to the config or update firmware, just powered off and back on.

Any ideas on how I can get this back online? I'm running IPO version 11.1.3.1.0 b34.

FWIW, I tested the serial connection and it does not respond, whereas the other analog modules reply to an AT command with Hello
 
Just replaced a Phone 30 presenting the same symptoms at a nursing home. Customer changed a UPS and it never came back despite efforts.
 
Thanks all. I swapped PSUs with another unit and the issue followed the PSU. The failed one showed 24v on a multimeter, but dropped to 0v on a 2 ohm load. Known good PSU dropped to 14v as a reference. Found a replacement 24v supply and we're back in business. I ordered a second one to have on hand.
