Amortization Schedule

I would like to create an amortization schedule based on parameters provided in a form (loan amount, rate term, etc.). I'm not sure how to get started with the queries, etc. Could someone provide some assistance or possibly email me with an example?

There are so many amortization calculators available these days, why would you want to do that to yourself? ;) If you have one, maybe you could provide a link to the calculator in a form.
