All Circuits Busy, BCM450 with full PRI

incoming calls are not working properly at all! I have a new BCM450 with a PRI and I get times when callers can't call in at all, not on a DID, or BTN or nothing. Oubound is fine always. I can watch BCM monitor and I see channel 5 almost flashing. If a call is on channel 1 and 2, then the next call skips a few channels and flashes on 5 and will not connect. PRI is provisioned normally.
 
Simple things first.
warm reset telephony..channels could be locked up on card

or reboot system.
 
should clocking be internal, or secondary external? Would this matter?
 
Is this the only PRi if so then it should be set as Primary to the carrier.
 
I am looking at our other PRI, and I have 2 settings different, Clocking and B channel selection. Should it be descending sequential, or assending sequential?

Normally, Descending Sequential, you need to be the opposite of telco.

Make the new PRI as Secondary External Clock this way if something happens to the Primary you have a back-up.
 
Have you installed the patch below, it corrects a number of PRI issues

I did install that, and come to think of it, the issues started around that time.
