Account is disabled even if password threshold not reached.

I have a single account that is locked out without reaching the password lockout threshold. This occurs consistently throughout each day. I have recreated the account and a host of other troubleshooting methods to no avail. This user account lives in a Windows 2000 domain. Interestingly, this is the ONLY account that has this problem.
 
Can you enable auditing on you domain controllers? Then you might be able to look in the event log see what account activity is going on. I had a problem similar to this once and it was due to an old server that was using that account for one of its services. The service would repeatidly try to start causing the account to lock.
